What you would do:
Alexander Youth Network is seeking Licensed Clinicians (LCSW, LCMHC, LMFT, or LCAS) for their brand new, state-of-the-art Facility Based Crisis program in Guilford County opening Spring of 2021. The Clinician plans, delivers, and monitors all clinical services for a caseload of children for a Facility Based Crisis program setting. Will be responsible for all clinical services delivered by the treatment team; leads the Core Assessment process and ensures follow-through on treatment recommendations; provides ongoing direct clinical services via individual sessions and family sessions; discharge planning and coordination, treatment team and weekly staffing meetings; May provide training and/or supervision on the NMT model to direct care staff and outside team members.. Full time, 2nd and evening/weekend shifts or On-call shifts available.
The Facility-Based Crisis program is a 16 bed inpatient service for children and adolescents that provides an alternative to hospitalization for youth presenting with escalated behavior due to a mental health, intellectual or developmental disability or substance use disorder who require treatment in a 24-hour residential facility. Facility-Based Crisis is an intensive short term, medically supervised service provided in a physically secure setting. It is available 24 hours a day, seven days a week, 365 days a year, with an expected length of stay of five to seven days.
ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE BUT NOT LIMITED TO:
- Provides quality services in accordance with NMT model and agency policies
- Completes the Psychosocial Assessment and Integrative Assessment
- All documentation including progress notes are completed accurately and in a timely manner
- Maintains active service plan on each client
- Accurately maintains client Electronic Health Record
- Completes all mandated training
- Maintains availability in clinical calendar
- Provides regular and consistent therapy
- Successfully meets discharge outcomes
EDUCATION and/or EXPERIENCE:
Masters degree and 1 year experience with the population served. Must be a NC licensed clinician (Fully or Provisionally Licensed). Proficiency in Microsoft Office applications.
CERTIFICATES, LICENSES, REGISTRATIONS
LCSW, LCMHC, LMFT, or LCAS or provisionally licensed and a N.C. Driver’s license
